About Our Provider Portal

Through our provider portal, primary care providers (PCPs) have an efficient tool to help manage care and outcomes for our most complex members. As a vital component of a member's medical home, you need the ability to view medical information for your patients.

Our provider portal gives designated PCPs access to important and timely member medical utilization information. It also allows you to access specific medical information and actively participate in a member's care plan.

You can view comprehensive medical utilization information, which is valuable in understanding what is going on with your patient.

You will save time by:

  • Having the treatment plan in one location
  • Being able to view services the member is using

Included information

Our provider portal includes a member profile, which is a rolling 12-month history reflecting all providers or services used by the member. General information is included, such as:

  • Primary and secondary diagnoses
  • Inpatient admissions
  • Emergency room utilization
  • Pharmaceutical history by drug classification

You'll also be able to use advanced features, such as:

  • Predictive modeling history
  • Claims history

How do I sign up?

Signing up is quick and easy:

  1. Visit the "Register" page to download, complete and sign the Web Portal Agreement.
  2. Return the agreement by faxing to (number) OR mailing to (address).

 

Once received, we will send you an email with your username and password. You'll have immediate access to those members' medical records.
