A care manager is someone who will work with you to help you stay well. They will help you work with your doctors to get what you need. A care manager will help you learn about your health and medicine. Knowing about your health can help you to take charge of your healthcare needs. All of this may improve your quality of life.

What happens when I join this program?

There is no cost to you for care management. MaineCare provides this service for you. You should keep seeing your PCP (Primary Care Provider). Always call your PCP if you are sick or have a health emergency.

What is an HRQ and why do I need one?

An HRQ is a "Health Risk Questionnaire." This is a screening tool that shows us how we can help you to improve your health.

What do I have to do to join this program?

What is a disclosure authorization form and why do I sign it?

This form is used to give us permission to talk to your providers about your health.
