Members
If
you are a MaineCare member and you have a health problem, we can help you. Some
examples of health problems are high blood pressure, heart disease, lung disease,
asthma and diabetes.
When you join our program, we can help you:
- Find help in your area
- Find a doctor
- Work with your other case managers
- Get to doctor visits
- Learn more about your medicines
- Learn more about how to stay healthy
- Get information about your condition
- Sign up for mail order pharmacy, so your medicines will come to you
How did I qualify?
You completed a screening with questions about your health. You may have been contacted
by
phone or mail to do this. Based on this health screening, you qualified for this
benefit because:
- You have one or more chronic conditions like
diabetes and high blood pressure; or
- You have been in the hospital or emergency room
often in the last year; or
- You have a disability; or
- You take lots of medicine.
How will I know who I am talking to?
- We will identify ourselves.
- Our name is Schaller Anderson.
- We are contracted with MaineCare.
- We will not ask you for money.
- We will not ask you for your Social Security number.
What will this benefit do for me?
- Help you get a primary care physician (PCP) if you don’t have one
- Work with you and your PCP to improve your health
- Help you find resources and services in your community
- Teach you about your personal health needs
We'll call you to see how you're doing and if there is anything you need us to help
you with.
Your current MaineCare benefits are not being changed in any way. This is an addition
to your
benefits. Our Care Management program is optional. You may opt-out of the Care Management
benefit at any time. If you would like to opt out of the benefit, you will need to
contact us by
