MaineCare’s free Care Coordination benefit assists members in reducing the frequency and severity of exacerbations of a chronic illness by improving the members’ health status and helping them to appropriately self-manage their conditions. Our benefit is developed using nationally recognized evidence-based guidelines that can be found on the menu bar.
The care coordination benefit has six components:
- Member identification processes
- Evidence-based practice guidelines
- Collaborative practice models that include physician and support-service providers
- Member self-care management education
- Process and outcomes measurement, evaluation and management
- Routine reporting/feedback loop (including communication with members, physicians, ancillary providers and provider profiling)
Members are identified as candidates for the free Care Coordination benefit through a stratification process using predictive modeling. If a member is stratified and placed in the high-risk category, a case manager contacts the member and completes an in-home assessment to health care needs or barriers to accessing health services.
Keeping members healthy is very important. Schaller Anderson is offering primary care physicians
a convenient and secure login in order to provide you with information to assist you in managing
your members’ health care. The secure login will provide you and your office representative
with access to the following:
- Review your members health profile
- Care plans of members participating in case management
- Feedback on care plan goals directly to assigned case managers
